was the flop call a mistake? should hero give up OTR? jam the river?

You block hearts. Most V’s aren’t folding middle overpairs+ for 1/2 PSB

What are you trying to fold out?

Trying to fold AK (we block), A high hands,
would jamming the river be a better paly?

Develop your post: your strategy, your range, the expected range for V, your equity and estimated EV. Do your homework.
Then I'd do the same for my answer.

I think the best play is to check behind. He has “shown you twice” that he wants to call. Any pair is calling the river for a decent sized bet, especially ones that 3 bet oop pre. You are basically betting to specifically get AhQh to fold (assuming a tighter 3 bet range)

What do you think about raise, flop bet turn, jam river line? We have all sets, also pre, I did not expect SB to 4! Hanse flatting BBs 3bet

You block part of the range you want him to fold with hearts. Jamming the river will be folded to less as he has less heart combinations to fold.

You don’t have to win every pot.
